Understanding your ID card.

Your member ID card carries everything a doctor's office or pharmacy needs to verify your coverage. The exact layout depends on your plan's network structure — find the one that matches your card below.

Only the subscriber and spouse receive printed cards. Anyone on the plan can print a temporary card or request new ones anytime from the member portal.

Single-network plans

One network, one set of copays.

If your plan uses a single national network (for example, Cigna PPO), your card shows one Benefit Networks block with your copays, your pharmacy (Rx) details, and the claims address on the back. Most members have a card like this.

Tiered-network plans

Two tiers, two networks.

If your plan uses a tiered network (for example, Tier 1: Northwell Direct and Tier 2: Anthem), the back of your card lists each tier separately with its own copays, deductible, and claims instructions. Check which tier a provider falls under before you receive care.
